Common use of Medical Certification of Serious Health Condition Clause in Contracts

Medical Certification of Serious Health Condition. 1. The unit member shall provide to the District medical certification of the serious health condition of a child, spouse or parent and a statement that the serious health condition requires the participation of a family member to provide care. 2. The unit member shall provide to the District medical certification of his/her own serious health condition and the inability to perform the functions of his/her position. 3. Medical certification may be provided by a physician, osteopath or other health care provider designated by the Secretary of Labor. (Form available from Human Resources) 4. The District may, at its expense, require additional medical evaluation and certification of the unit member's own serious health condition (but not of the unit member's spouse, parent or child). 5. The District shall require release to return to work from an employee’s health care provider following leave for the unit member's own serious health condition.
