Nicotine Incentive. Employees who participate in Healthy by Choice and receive the incentive for non-tobacco use are required to sign a confirmation form attesting that the Employee has not used any tobacco products during the prior calendar year. In the event that the Employee uses tobacco, the Employee may make a request for an alternative standard during open enrollment and the City will work with an employee (and, if they wish, the employee’s doctor) to earn the same reward by different means. Any Employee who falsifies an attestation regarding non-tobacco use may be subject to disciplinary action.
