Notice of Placement in File. An employee shall be notified when an evaluation, commendation, or record of disciplinary action is placed in his/her personnel file. A copy of an evaluation, commendation, or record of disciplinary action shall be made accessible to the employee within a reasonable time after it is placed in the employee’s personnel file. No citizen’s complaint shall be placed in an employee’s personnel file unless the complaint is accompanied by a specific disciplinary action related to it.
