Personal Health Information (PHI) means individually identifiable health information 21 usually transmitted by electronic media, maintained in any medium as defined in the regulations, or for 22 an entity such as a health plan, transmitted or maintained in any other medium. It is created or received 23 by a covered entity and relates to the past, present, or future physical or mental health or condition of an 24 individual, provision of health care to an individual, or the past, present, or future payment for health 25 care provided to an individual.
