Common use of Personal Physician Clause in Contracts

Personal Physician. If an employee wishes to be treated by a personal physician(s) or medical facility selected pursuant to Labor Code Section 4600, the employee shall notify the District in writing (see Appendix “I”), of the name and address of such personal physician(s) or medical facility prior to injury.

Personal Physician. If an employee a bargaining unit member wishes to be treated by a personal physician(s) or medical facility within a reasonable geographic area selected pursuant to Labor Code Section 4600, the employee they shall notify the District in writing (see Appendix “I”), of the name and address of such personal physician(s) or medical facility prior to injuryfacility.
