Physical/Occupational Therapy. Physical and occupational therapy is covered only when: a program is implemented to restore or attain a higher level of independent functioning or new skills in the most timely manner possible; physical or occupational therapy is received from a licensed physical or occupational therapist; physical or occupational therapy is ordered by a doctor; the therapy will result in significant, sustained measurable functional or skill status given your condition; and such improvement will not diminish with the removal of the therapeutic agent or environment. Preauthorization is recommended for the eleventh and subsequent visit. Inpatient Medically necessary inpatient physical or occupational therapy is covered as a hospital service. See Section 8.0.
