The Physician Statements clause requires a physician to provide specific written declarations or certifications regarding a patient's medical condition or treatment. Physician Statements. Employees with an illness or disability exceeding three (3) days, shall be required to furnish a statement from his/her physician, notifying the County that the employee is unable to perform his/her duties, and shall be required to present a statement from his/her physician upon his return to work, indicating his/her fitness.
