Plan Changes. If the employer maintains a written Plan of benefits, the name of such Plan is shown on the first page of this contract. This contract applies to the terms of the Plan in effect on the Effective Date and to each Plan change if Prudential consents. The Contract-Holder will furnish Prudential with a copy of the Plan. While this contract is active, the Contract-Holder will also furnish a copy of each Plan change.
