Medical and Insurance Benefits. You will be entitled to receive all employee benefits provided to Torotel employees generally from time to time, so long as and to the extent the same exist.
Medical and Insurance Benefits. During the term of this --------------------------------------------- Agreement, the Employee shall be entitled to health and hospitalization insurance and other benefits in at least the same amounts and with the same benefits as Employee received from Employer immediately prior to the effective date of this Agreement or such additional amounts and/or benefits as Employer may furnish to the other employees and executives of the Employer from time to time during the term of this Agreement. Employee shall be entitled to participate in and receive the benefits of any plan of the Employer relating to hospital, surgical, medical and dental coverage or reimbursements; group life insurance, accidental death and dismemberment, and disability insurance; or any other plans, benefits and privileges, together with such other and additional fringe benefits and such improvements to existing benefits as the Employer may make available to the employees and executives of the Employer, now existing or which may hereafter come into existence during the term of this Agreement.
Medical and Insurance Benefits. During the Term, the Company shall provide Executive with those group medical, health insurance, disability insurance and life insurance benefits generally available to its senior executive officers, as such benefits may be modified from time to time in the Company's sole and absolute discretion.
Medical and Insurance Benefits. 13.1 Employees will be included in the same Public Employees Benefit Board authorized state- employee benefits and the same wellness plans as analogous employees at UW Medical Center - Montlake. No other benefit plans shall be offered to Employees.
a. For the 2019-2021 biennium, the Employer will contribute an amount equal to eighty-five percent (85%) of the total weighted average of the projected medical premium for each bargaining unit employee eligible for insurance each month, as determined by the Public Employees Benefits Board (PEBB). The projected medical premium is the weighted average across all plans, across all tiers.
b. The point-of-service costs of the Classic Uniform Medical Plan (deductible, out-of-pocket maximums and co-insurance/co-payment) may not be changed for the purpose of shifting health care costs to plan participants, but may be changed from the 2014 plan under two (2) circumstances.
1. In ways to support value-based benefits designs; and
2. To comply with or manage the impacts of federal mandates. Value-based benefits designs will:
1. Be designed to achieve higher quality, lower aggregate health care services cost (as opposed to plan costs);
2. Use clinical evidence; and
3. Be the decision of the PEBB Board.
c. Article 46.1 (B) will expire June 30, 2021.

b. If the PEB Board authorizes stand-alone vision insurance coverage, then the Employer will pay the entire premium costs for each bargaining unit employee.

12.1 Employees in the NWH Bargaining Unit will be included in the same Public Employees Benefit Board authorized state-employee benefits and the same wellness plans as employees at UW Medical Center in analogous employee types. Employees will no longer receive a premium or credit for waiving any elements of coverage.
12.2 For the 2019-2021 biennium, the Employer will contribute an amount equal to eighty-five percent (85%) of the total weighted average of the projected health care premium for each bargaining unit employee eligible for insurance each month, as determined by the Public Employees Benefits Board. The projected health care premium is the weighted average across all plans, across all tiers.

12.5 The PEBB Program shall provide information on the Employer Sponsored Insurance Premium Payment Program on its website and in an open enrollment publication annually.
12.6 The Employer will pay the entire premium costs for each bargaining unit employee for basic life, basic long-term disability and dental insurance coverage.
12.7 Wellness
12.7.1 To support the statewide goal for a healthy and productive workforce, employees are encouraged to participate in a Well-Being Assessment survey. Employees will be granted work time and may use a state computer to complete the survey.
12.7.2 The Coalition of Unions agrees to partner with the Employer to educate their members on the wellness program and encourage participation. Eligible, enrolled subscribers who register for the Smart Health Program and complete the Well-Being Assessment will be eligible to receive a twenty-five dollar ($25) gift certificate. In addition, eligible, enrolled subscribers shall have the option to earn an annual one hundred twenty-five dollars ($125.00) or more wellness incentive in the form of reduction in deductible or deposit...
Medical and Insurance Benefits. 14.1 Medical, Dental and Insurance. Regular full-time employees and regular part-time employees are eligible for medical, dental and vision insurance benefits. Any increases in premiums, if any, after Plan Year 2017, shall be borne equally by the Employer and the employee through payroll deduction. In no event shall employees’ premiums increase by more than 2% in each year for the employee-only level of insurance, and 4% for each other level of insurance and dependent coverage. In the event that a premium decreases, 50% of the decrease shall be allocated to the employee’s share of the premium. Participation in medical and dental benefits shall be subject to the enrollment and eligibility requirements of the specific plan. See Addendum A for premium contributions.
14.1.1 Should the Employer change plans, the Employer shall notify the Association 45 days prior to implementation and meet with the Association upon request.
14.1.2 At least one registered nurse and one licensed practical nurse shall participate in reviewing the annual bids for employee medical insurance.
