Post Incident Review Report. The Contractor must provide a Post Incident Review Report following a major Event that affects TMRN performance, causes a Fault or results in disruption to the TMRN. The Post Incident Review Report must be provided within five Business Days of a major Event. The Post Incident Review Report must contain details of the incident together with the actions taken as well as detailing what actions will be taken to prevent re- occurrences and/or assist with the resolution should it happen again in the future. In addition, the Event process as a whole must be reviewed and improvements identified. The Post Incident Review Report must also contain actions identified, who has been assigned the action and the expected completion date of the actions. It is the responsibility of the Contractor to ensure the actions are completed accordingly.
