Preparation of the Site Investigation Report. A hydrogeological and geotechnical site investigation (HGI) report will be prepared to summarize the results of the field investigation program. The report will be prepared as a component of the permit application, as required by Rule 62-701.330(3) of the F.A.C. A subsurface hydrogeological and geotechnical model will be developed for use in evaluating landfill geotechnical stability, and preparation of the FDEP solid waste permit and ERP applications. The HGI report will address the following hydrogeological requirements set forth in Rule 62- 701.410(1) of the F.A.C.: • Description of the landfill site geology and hydrogeology and its relationship to the local and regional hydrogeologic patterns; • Inventory of public and private water xxxxx within one-mile radius of the proposed landfill site; and • Map showing the location of potable xxxxx within 500 feet of the landfill, and locations of xxxxx serving community water supplies within 1,000 feet of the proposed landfill development. The HGI report will also address the following geotechnical requirements set forth in Rules 62- 701.410(2) and (3) of the F.A.C.: • Interpretation of the subsurface conditions including soil stratigraphy and groundwater table conditions; • Potential presence of muck, previously filled areas, soft ground, lineaments, and sinkholes; • Potential fault areas, seismic impact zones, and unstable areas; • Estimates of average and maximum high groundwater table across the site; and • Foundation analysis to evaluate the ability of the foundation to support the loads and stresses imposed by the proposed landfill development.

General Law § 27-20-60. An approved clinical trial is a phase I, phase II, phase III, or phase IV clinical trial that is being performed to prevent, detect or treat cancer or a life-threatening disease or condition. In order to qualify, the clinical trial must be: • federally funded; • conducted under an investigational new drug application reviewed by the Food and Drug Administration (FDA); or • a drug trial that is exempt from having such an investigational new drug application. To qualify to participate in a clinical trial: • you must be determined to be eligible, according to the trial protocol; • a network provider must have concluded that your participation would be appropriate; and • medical and scientific information must have been provided establishing that your participation in the clinical trial would be appropriate. If a network provider is participating in a clinical trial, and the trial is being conducted in the state in which you reside, you may be required to participate in the trial through the network provider. Coverage under this plan includes routine patient costs for covered healthcare services furnished in connection with participation in a clinical trial. The amount you pay is based on the type of service you receive. Coverage for clinical trials does not include: • the investigational item, device, or service itself; • items or services provided solely to satisfy data collection and that are not used in the direct clinical management; or • a service that is clearly inconsistent with widely accepted standards of care.

  • Grievance Investigations Where an employee has asked or is obliged to be represented by the Institute in relation to the presentation of a grievance and an employee acting on behalf of the Institute wishes to discuss the grievance with that employee, the employee and the representative of the employee will, where operational requirements permit, be given reasonable leave with pay for this purpose when the discussion takes place in the headquarters area of such employee and leave without pay when it takes place outside the headquarters area of such employee.

  • Cooperation with Investigations You agree to cooperate with us in the investigation of unusual transactions, poor quality transmissions, and resolution of customer claims, including by providing, upon request and without further cost, any originals or copies of items deposited through the Service in your possession and your records relating to such items and transmissions.

  • Evaluation Report 20.5.4.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and administrative evaluator(s) and shall include each evaluator’s individual rating. The summary evaluation report shall take into account the results of each of the evaluation components (Section 20.5.3) in order to arrive at an overall rating. When the committee and the administrative evaluator(s) cannot reach an agreement as to the overall rating, the report must include written explanation.
