Procedures - Serious Incidents. (i) The Centre/Club Manager must immediately notify their Senior Manager and the WHS Manager if the following occurs:  a person needing immediate medical treatment for broken bones, loss of a bodily function (eg loss of consciousness, heart attack) separation of skin from underlying tissue, such as de-gloving or scalping,  An ambulance is called and the person transported to hospital for treatment.  Medical treatment for exposure to a hazardous substance (such as chemicals or a biological material).  If there is a dangerous occurrence (e.g. major chemical spill or biological exposure) that creates a risk to persons in the vicinity or the environment. If the WHS Manager is unable to be contacted the Senior Manager will notify the authority.

  • Data Incidents Merchant must report all instances of a Data Incident (as defined in the American Express Merchant Operating Guide) immediately to ISO after discovery of the incident. Merchant must ensure data quality and that Transaction Data and customer information is processed promptly, accurately, and completely, and complies with the American Express Technical Specifications.   • DISCIPLINE PROCEDURES The discipline procedure may be initiated only within twenty (20) Days of the date the President or Vice-President (Academic and Research) knew, or ought reasonably to have known, of the occurrence of the matter giving rise to discipline. The Employer shall have the right to request, in writing to the Union, an extension of ten (10) Days. The Union shall not unreasonably reject the Employer’s request.

  • Right to Grieve Disciplinary Action Employees shall have the right to grieve written censures or warnings, and adverse employee appraisals. Employees shall have the right to rebut in writing any disciplinary notice and that rebuttal will be placed in the employee file, but will not be part of the formal disciplinary record. Should an employee dispute any such entry in his/her file, he/she shall be entitled to recourse through the Grievance Procedure and the eventual resolution thereof shall become part of his/her personal record.
