Professionalism Form. The department chair, faculty leader, or designee will complete the Professionalism Form before the end of the twelfth week of the Evaluatee’s fourth Fall Semester.

Professionalism Form. The academic administrator responsible for the Emeritus College shall submit any information with supporting evidence related to the Professionalism Form to the Evaluator. This information will be submitted to the Evaluator, following the completion of the Faculty Observation Form and prior to the completion of the Summary Form. The Evaluator will complete the Professionalism Form using the information received from the academic administrator responsible for the Emeritus College and will submit a copy of this form to the academic administrator responsible for the Emeritus College before meeting with the evaluatee.
