Common use of Province of Ontario Medicare Clause in Contracts

Province of Ontario Medicare. Programme, or same coverage, and Major Medical coverage for an employee and his eligible dependents shall commence as soon as he has completed his probationary period with the Company and shall terminate on the last day of the calendar month during which he last earned wages. Except that insurance coverage for an employee (including an employee on sick leave) who is laid off pursuant to Article IX hereof, shall terminate after thirty (30) days have elapsed from date of his separation by layoff or from the last day for which he was paid in lieu of notice of layoff. In the event of an employee’s death, his coverage shall terminate immediately after two (2) clear months have elapsed from date of his death. SICKNESS/ACCIDENT INSURANCE
