Purpose and Goals. The Garfield Wellness & Recovery Center is a long-term Social Rehabilitation Facility (SRF) providing support and recovery services in a homelike environment. The facility is developed for the purpose of providing non-medical treatment and rehabilitation services to 18 to 59-year-old adults with significant, chronic, mental health challenges. The SRF will provide rehabilitation services that will help reduce clients’ readmissions or return to acute care facilities, hospitals, or institutions. The goal of the SRF program is to assist clients in reentering and functioning independently in their respective communities in 18 months or less. Each client will be made to feel at home in the facility. The client, the client’s representative, family member (where applicable), a licensed mental health professional, and the Program Director will collaboratively develop an individual Treatment Plan that will expedite recovery and the client’s readiness to function independently when he or she reenters the community. The client’s progress towards the goals in the plan will be evaluated on a monthly basis by a licensed mental health professional. The program will include services designed to assist persons considered seriously disabled due to a mental illness to develop skills to become self-sufficient and capable of increasing levels of independent functioning in the community. The services provided include non-medical psychiatric and psychological services, educational services, prevocational and vocational counseling, development of independent living skills, self-help and social skills, and community outreach to develop linkages with other support and service systems, including family members. All services shall be client-centered, in recognition of varying individual goals, diverse needs, concerns, strengths, motivations, and challenges. The program ensures the participation of clients in all aspects of the program, and plan development will include individual needs and services planning, program design and evaluation. The SRF program includes structured day and evening services that are described in the Range of Services section of the Program Plan.
