QJSA Information to a Participant. No more than 90 days before the Annuity Starting Date, each Participant who is eligible for an annuity form of payment shall be given a written explanation of (1) the terms and conditions of the QJSA, (2) the right to a period of at least 30 days after receipt of the written explanation to make an election to waive this form of payment and choose an optional form of payment and the effect of this election, (3) the right to revoke this election and the effect of this revocation, and (4) the need for Spousal Consent. --------------------------------------------------------------------------------
