Replacement Card Fee. If you request a replacement Card you will be charged a replacement Card fee as indicated on our Fee Schedule.
Replacement Card Fee. We may charge you a fee of up to $5 for a replacement card request.
Replacement Card Fee. The Membership Card deposit as per the Membership Registration & Agreement Form, is refundable on return of the Membership Card. If a Membership Card is lost, destroyed or damaged and requires replacement a Replacement Card is available at an NBRC Administration Centre for an additional $10.00, non-refundable deposit.

I understand and acknowledge that I shall be liable for all losses suffered by CCB (Asia) if I (and/or any supplementary Cardmembers(s)) have acted fraudulently or with gross negligence.
Replacement Card Fee. We may charge a $5.00 replacement fee for each card issued after the third card in a 12-month period. The replacement fee does not apply when we replace a card upon its expiration and/or due to identity theft
