Report to Superintendent. Within fifteen (15) calendar days after completion of the probation period, the evaluator shall submit a written report to the superintendent, on each probationary employee, which report shall: a. Specify the number of observations; b. Include all evaluation forms utilized in the evaluation process; c. Include a recommendation as to the employee’s future employment status; and d. Be duplicated and copies given to the evaluator, the employee, and the Association if involved.
