REPORTING, EVALUATION, AND MONITORING REQUIREMENTS. Notwithstanding the reporting requirements set forth herein and subject to paragraph 21, below, if Provider and the Mental Health Board otherwise agree in writing to an alternate method of reporting or an alternate format for reporting, that alternate agreed upon method will control. Provider shall track and report measurable outcome data, service information, evaluation and monitoring data, and the agency’s emergency response plan as specified below:
