Measurable Outcomes. 1. To be submitted in accordance with the Work Plan, reporting on domains selected on the funding application. 2. Pursuant to the Work Plan, unless otherwise authorized by the Mental Health Board, all funded programs will adopt measurable outcomes. Outcome reports are required to be completed using mechanisms and timeframes as set forth by the Mental Health Board. Outcome reports shall be completed and submitted to the Mental Health Board in the specified format by the last day of the month following the end date of each reporting period. 3. Provider is required to maintain the use of the assessment tool(s) identified on the Work Plan to measure outcome domains. 4. In order to ensure accuracy and continuity for outcomes reporting and persons served each quarter, Provider must make available at time of compliance audit, written documentation of the process or procedure by which the Provider measures, accumulates, calculates and reports applicable data, including unduplicated persons served as required per Work Plan.

Measurable Outcomes. A Recipient Program or program receiving funding from the Clean Water Fund must meet or exceed the constitutional requirement to protect, enhance, and restore water quality in lakes, rivers and streams and to protect groundwater and drinking water from degradation. A Recipient Program or program receiving funding from the Clean Water Fund must include measurable outcomes, as defined in Minn. Stat. § 3.303, subdivision 10, and a plan for measuring and evaluating the results. A Recipient Program or program must be consistent with current science and incorporate state-of-the-art technology. All information for funded Recipient Program work, including the proposed measurable outcomes, must be made available for publication on the web site required under Minn. Stat. § 3.303, subdivision 10, as soon as practicable and forwarded to the Met Council and the Legislative Coordinating Commission under the provisions of Minn. Stat. § 3.303, subd. 10. The Recipient must compile and submit all information for funded Recipient Programs or programs, including the proposed measurable outcomes and all other items required under Minn. Stat. § 3.303, subdivision 10, to the Met Council and, if requested by the Met Council, the Legislative Coordinating Commission as soon as practicable or by January 15 of the applicable fiscal year, whichever comes first.

One hundred percent (100%) of families who report moderate to high stress levels on the quarterly parent survey will be referred to the Strong Beginnings Social Worker for further evaluation. 7.5. Sixty percent (60%) of participating families will attend at least one (1) group connection gathering quarterly. A) Sixty percent (60%) will connect to the resources. 7.6. Eighty percent (80%) of participating families will be provided with at least one (1) resource connection quarterly. 7.7. Sixty percent (60%) will connect to the resources. 7.8. Ninety percent (90%) of clients referred will be contacted within five (5) business days. 7.9. Ninety percent (90%) of referred clients will have contact attempted to schedule appointment to enroll them in the program within ten (10) business days.
