Reports of Security Incidents. Beginning as soon as practicable but in no event later than the effective date of the Security Rule, Business Associate shall report to the County Director within two (2) days of discovery any security incident of which it becomes aware.

Reports of Security Incidents. In addition to following the breach notification requirements in section 13402 of the Health Information Technology for Economic and Clinical Health Act of 2009 (“HITECH Act”) and related regulations and guidance, Business Associate shall report to [Insert Agency Name]’s Privacy Officer, or his designee, within two (2) days of discovery any security incident of which it becomes aware. At the sole expense of Business Associate, Business Associate will comply with all applicable federal and state breach notification requirements.
Reports of Security Incidents. Business Associate hereby agrees that it shall notify, in writing, the Department’s Project Officer within two (2) days of discovery of any Security Incident at the time of Business Associate’s notification to Covered Entity or promptly thereafter as such information becomes available.
