Request and Approval. When the appropriate administrator becomes aware that an employee has taken or intends to take time off for an FML qualifying reason pursuant to provision 28.3, the employee may be asked to provide acceptable documentation from a medical professional asserting that there is an FML qualifying reason. FML qualifying leaves may be designated as FML.
Request and Approval. Use of accrued sick leave for personal leave and religious holidays shall be granted only with the prior approval of the supervisor. Application should be made to the supervisor at least 48 hours in advance.

Except as otherwise provided in this Agreement, written requests for leave of absence shall be made by employees prior to the beginning of the period(s) of absence and no payment for any absence shall be made until the leave is properly approved. All leaves of absence without pay shall be granted at the discretion of the Employer and must be approved by the Employer in advance (except as provided for under FMLA). Upon application by the employee, leaves of absence may be extended or renewed at the discretion of the Employer (except as provided for under FMLA). Request and Approval. Department Directors are responsible for arranging leave schedules so that adequate personnel are available to carry on necessary City work. The use of general leave shall require approval of the Department Director or his/her designee. Whenever possible, general leave requests must be approved in advance of the days to be taken as general leave. The procedure for requesting the use of general leave shall be as follows:

However, in emergency situations, vacation time may be granted at the employee’s supervisor’s discretion with less than the seven (7) calendar day advance request. FML qualifying leaves may be designated as FML.
Request and Approval. 6.3.3.1 All vacation time must be requested at least seven (7) calendar days in advance of use. However, in emergency situations, vacation time may be granted at the employee’s supervisor’s discretion with less than the seven (7) calendar day advance request. This exception can only occur up to two (2) times per calendar year. Vacation leave will be granted on a first request basis and will normally conform to program operation schedules. Vacation time or personal leave will not be approved when four (4) or more employees have already requested vacation or personal leave for the same day(s). In situations reasonably deemed by the SSA Director/designee to be emergencies, the SSA Director/designee shall approve vacation time or personal leave.
6.3.3.2 Vacation shall be requested in writing on the request form provided by the Board.
Request and Approval. A standard Leave of Absence Request form stating the specific reason for the request must be submitted to the supervisor. All leaves of absence without pay must also be approved in advance by the Director and/or the Board of Trustees. Requests for leaves of absence by employees who are scheduled to work less than 1,250 hours per year for those reasons listed in the Personnel Policy Manual under Family and Medical Leave shall be granted a leave of absence not to exceed twelve (12) weeks. However, this leave is not Family and Medical Leave under either the FMLA or Article 30 and the employee must comply with Library policies regarding leaves of absence.
