Request for an Unpaid Medical Leave of Absence. A request for an Unpaid Medical Leave of Absence shall be submitted by following procedures set forth by the School Board and/or the Leave of Absence Request Form (if 11 days or more) with appropriate supporting documentation and information and will be approved or denied on an individual basis.
