Unpaid Medical Leave of Absence. Employees who have exhausted earned paid absence leave, may request an Unpaid Medical Leave of Absence not to exceed six (6) calendar months.
Unpaid Medical Leave of Absence. Benefit eligible/full-time employees, who have exhausted earned paid absence leave, may request an Unpaid Medical Leave of Absence not to exceed 6 calendar months.
Subd 1. An Unpaid Medical Leave of Absence, when granted, shall be for a period of time as is agreed upon between the Director of Human Resources and the employee, subject to the approval of the School Board. An Unpaid Medical Leave of Absence shall not be granted to secure other employment.
Unpaid Medical Leave of Absence. (a) The Employer shall comply with all provisions of the Family and Medical Leave Act and California law regarding medical leaves. The Employer also, at its discretion, may grant additional leave under this provision and will consider doing so if the employee is disabled by cancer or other life-threatening illness.
(b) Benefits accumulated prior to leave of absence without pay shall be retained but will not continue to accrue during such leave.
Unpaid Medical Leave of Absence. An employee can request a leave of absence if the employee suffers a serious illness or injury, or if a member of the employee's family (spouse, child, and parent or legal guardian) suffers an illness or injury requiring the employee's supervision and care. The employee must use their accumulated sick leave whether they are eligible for FMLA, (Family Medical Leave Act), or not. Medical leaves will typically be granted for up to one year. Such leaves can be extended at the discretion of the School Board.
Unpaid Medical Leave of Absence a. Upon written request of a bargaining unit member to the Board, the member shall be granted a leave of absence for illness or other disability in accordance with Ohio Revised Code 3319.
Unpaid Medical Leave of Absence. A. An employee shall be granted a leave of absence, without pay, for a period not to exceed one (1) year because of personal illness or injury supported by medical evidence. If the illness or injury continues beyond one (1) year, the Board, upon request of the employee, may grant an additional one (1) year leave. Upon the intended return of the employee, the employee shall supply the Board with an attending physician’s statement indicating the employee’s ability to return to full duties. Such statement shall be submitted to the superintendent’s office as soon as it is available. The employee shall then return to the job in twenty (20) working days or less (as determined by the superintendent).
B. Employees on approved unpaid medical leaves of absence may maintain their hospitalization, dental insurance coverage, and other coverages by paying the Treasurer in advance monthly, the full cost of the premium, including the share normally paid by the Board.
Unpaid Medical Leave of Absence. Full-time employees may request an Unpaid Medical Leave of Absence not to exceed one (1) year of service. An employee's position may be posted temporarily pending their return, subject to the best interests of the School District. An employee returning from an unpaid medical leave shall be entitled to return to his/her former position.
Unpaid Medical Leave of Absence. Leaves of absence for illness or injury shall be granted without prior Employer approval to employees who have completed their probationary period and who have expended all accrued sick leave benefits; provided however, medical verification may be required. The maximum duration for medical leave shall be one (1) year. In the event the medical leave is related to a proven industrial related illness or injury the length of the leave shall be determined through consultation with the employee, the employee’s physician, and the employer. The employer reserves the right to require independent medical verification.
Unpaid Medical Leave of Absence. (a) After an employee has exhausted his sick leave with pay, he or she shall be granted a leave of absence without pay for a period not to exceed six (6) months because of personal illness, injury or pregnancy (including postpartum recovery period), upon written request, supported by medical evidence satisfactory to the City if the employee has reported such illness, injury or pregnancy (including postpartum recovery period), to his or her Department Head by no later than the second day of absence.
(b) If the illness, injury or pregnancy (including postpartum recovery period) continues beyond six (6) months, the City may grant additional leave under this provision, upon written request.
(c) An employee on an unpaid medical leave of absence is expected to keep the City informed on the progress of his or her illness, injury or pregnancy (including postpartum recovery period), as circumstances allow.
(d) Any employee who has been on an unpaid medical leave of absence beyond five (5) consecutive work days may be required to submit to and pass a physical examination before being permitted to return to work.
Unpaid Medical Leave of Absence. (a) An employee who sustains a job-related injury shall have the option of using his paid sick leave, paid vacation leave, unpaid medical leave of absence or workers' compensation (which shall not count as hours worked), whichever he prefers.
(b) After an employee has exhausted his sick leave with pay, he or she shall be granted a leave of absence without pay for a period not to exceed six (6) months because of personal illness, injury or pregnancy (including postpartum recovery period) upon written request, supported by medical evidence satisfactory to the City if the employee has reported such illness, injury or pregnancy (including postpartum recovery period) to his or her Department Head by no later than the second day of absence.
(c) If the illness, injury or pregnancy (including postpartum recovery period) continues beyond six (6) months, the Employer may grant additional leave under this provision, upon written request.
(d) An employee on an unpaid medical leave of absence is expected to keep the Employer informed on the progress of his or her illness, injury or pregnancy (including postpartum recovery period), as circumstances allow.
