Resident's Representations and Warranties. Resident represents and warrants that: Resident has never had his/her license to practice medicine revoked or suspended in any state. Resident shall secure and maintain a narcotics number issued by the appropriate government agency. There is no litigation, proceeding or governmental investigation, including, but not limited to, any malpractice, bankruptcy or exclusion proceeding, pending or threatened, against Resident, except as disclosed by Resident. Resident shall execute any agreements necessary for participation by SRHS and Resident in, and reimbursement from, public or private third-party payers including, but not limited to, Medicaid, Medicare, and other private managed care programs with which SRHS for itself or its employed physicians otherwise contract as providers.
