Revision to Application for Relief Work Form. Employees may revise their Application For Relief Work form two (2) times per year. March 15th to be effective April 15th and September 15th to become effective October 15th. The most current relief list shall remain posted at all times. In addition, Employees shall have fourteen (14) days following the start date of the change of circumstances, to make their revision to the Application for Relief Work Form under the following circumstances:
