SELF-SCORING WORKSHEET. The Offeror shall complete the Self-Scoring Worksheet (Attachment 4) for all Proficiency Areas within the Market Segment that the Offeror is submitting a proposal in accordance with the instructions below. Offerors shall not include any other additional information within the Self- Scoring Worksheet. 1. Fill in Offeror’s name. 2. Go to one or more of the Market Segments tabs that you are proposing on. 3. Fill in each Proficiency Area being proposed, whether information is from the Prime or Subcontractor/Teaming Partner. 4. If the answer to above is Subcontractor or Teaming Partner, then fill in their name. 5.
