Services provided by the Practice. 2.1 In consideration of the fees outlined in 1.1 above, the Practice agrees to provide the dental services described in clause 2.2 below either by the dentist or by a suitably qualified partner, member of staff, deputy or locum.

Services provided by the Practice. 2.1 In consideration of the fees outlined in 1.1 above, the Practice agrees to provide the services described in 2.2 below either through the Vet or by a suitably qualified partner, member of staff, deputy or locum. 2.2 The services to be provided for dogs over 30kg are: • Annual vaccination • Cats: Cat flu, Enteritis, Feline Leukaemia,Feline Chlamydia • Dogs: Distemper, Hepatitis, Leptospirosis, • Parvovirus, Parainfluenza • Flea treatment • Including lungworm and mites • Worming treatment • Including roundworms and tapeworms • Annual health assessment with our vet • Annual blood pressure monitoring • Six-monthly check-up with our nurses • Annual nail clip • Dental check/ Flea check • Nutrition advice • Other preventative care and behavioural nurse clinics (e.g. weight or senior pet) • Telephone advice • Free use of Bright Side Vets Early Doors Service • 10% off retail (food or toys) • 15% off routine operations (e.g. neutering,dental) 2.3 For the purposes of clause 2.2, 12 months will run from the Date of Commencement and each anniversary of that date. 2.4 The fees paid under this Agreement do not cover costs associated with treatment which has been specifically agreed to be excluded between you and your Vet, would be charged for separately.
Services provided by the Practice. 2.1 In consideration of the Fees outlined in 1.1 above, the Practice agrees to provide the Services described in 2.2 below either through the Dentist or by a suitably qualified partner, member of staff, deputy or locum. 2.2 The services to be provided are: • 4 x single hygiene appointments • 2 x check up appointments • X-rays if needed • 10% discount on any further hygiene visits needed throughout the yearAccident & Emergency Insurance 2.3 For the purpose of clause 2.2, 12 months will run from the Date of Commencement and each anniversary of that date. 2.4 The fees paid under this Agreement do not cover costs associated with treatment which has been specifically agreed to be excluded between you and your Dentist, these being: orthodontic appliance therapy, dental implants and related superstructures, treatment which is purely cosmetic, treatment carried out at another Practice (other than for temporary emergency treatment covered by the insurance as described in 2.2 above), pharmaceutical items, laboratory fees, prescription fees, extraction of wisdom teeth, sedation fees, or referral to specialists. These would be charged for separately.

This would be charged separately.
Services provided by the Practice. 2.1 In consideration of the Fees outlined in 1.1 above, the Practice agrees to provide the Services described in 2.2. 2.2 The Services to be provided to dogs up to 10kg in weight are: • Annual health check to include nail clipping and dental check • Annual vaccinations (excluding kennel cough) • Lungworm prevention • Year round worming and flea treatment • Microchip • 10% discount off neutering, dental treatment and additional consultations • Pet accident insurance 2.3 For the purpose of clause 2.2, 12 months will run from the Date of Commencement and each anniversary of that date. 2.4 The Fees paid under this Agreement do not cover costs associated with treatment which has been specifically agreed to be excluded between You and the Practice. This would be charged for separately.

These would be charged for separately.

These would be charged for separately.
