Statement of Services Offered by APRN. The APRN, in collaboration with the Physicians, will provide to the Practice’s patients preventative, diagnostic, therapeutic and primary care services in the APRN’s Specialty at multiple locations (See Exhibit A). The APRN will evaluate and promote wellness within the area of the APRN’s Specialty in a manner consistent with the APRN’s education and certification, as set forth in this SCA and in accordance with the Ohio Revised Code and the rules adopted by the Board. The APRN holds a valid APRN license and shall prescribe in collaboration with the Physicians. The APRN shall prescribe drugs and therapeutic devices in accordance with Sections 4723.481 to 4723.485 of the Ohio Revised Code and Section 4723-9-09 of the Ohio Administrative Code, and the most recent formulary established by the Board and the Committee on Prescriptive Governance (“Formulary”) pursuant to Section 4723.50 of the Ohio Revised Code.
