Common use of Supports Coordinator Clause in Contracts

Supports Coordinator. The Supports Coordinator (the LTSS Supports Coordinator or PIHP Supports Coordinator) is a member of the ICT who is available to Enrollees who have identified LTSS, behavioral health, intellectual/developmental disabilities, or substance use needs. The Supports Coordinator collaborates with the Enrollee and the ICO Care Coordinator to assure all necessary supports and services are provided to enable the Enrollee to achieve desired outcomes. Refer to Section 2.5.4 of this Contract for additional details specific to the LTSS Supports Coordinator.
