Common use of Termination of Subscriber Coverage Clause in Contracts

Termination of Subscriber Coverage. A Subscriber’s coverage will terminate for any of the following reasons: 1. employment terminates; 2. the Group Agreement terminates; 3. the Subscriber is no longer eligible as outlined in this Certificate and/or on the Schedule of Benefits; or 4. the Subscriber becomes covered under an alternative health benefit plan or under any other plan which is offered by, through, or in connection with, the Contract Holder in lieu of coverage under this Certificate.
