Toll-Free IVR. On or before the Notice Deadline, the Settlement Administrator shall establish a Toll-Free IVR phone number with script recordings of information about this Settlement, including information about 24 the Claim Form, utilizing the relevant portions of the language contained in 25 the Notice and Claim Form. The Toll-Free number shall remain open and accessible through the Claim Deadline. The Settlement Administrator shall 27 make reasonable provision for Class Counsel to be promptly advised of recorded messages left on the Toll-Free number by potential Settlement Class Case 3:17-cv-02335-GPC-MDD Document 232-3 Filed 11/08/19 PageID.10559 Page 24 of 84 members concerning the Litigation or the Settlement so that Class Counsel may timely and accurately respond to such inquiries; provided, however, the Settlement Administrator shall review the recorded messages before providing them to Class Counsel and if one or more of the messages requests a blank Claim Form or other similar administrative assistance only, then the 6 Settlement Administrator shall handle such administrative request(s), but the Settlement Administrator shall provide all other messages to Class Counsel for any further response to the Settlement Class member.

  • TOBACCO FREE CAMPUS All Orange County operations under the Board of County Commissioners shall be tobacco free. This policy shall apply to parking lots, parks, break areas and worksites. It is also applicable to Contractors and their personnel during contract performance on County owned property. Tobacco is defined as tobacco products including, but not limited to, cigars, cigarettes, e-cigarettes, pipes, chewing tobacco and snuff. Failure to abide by this policy may result in civil penalties levied under Chapter 386, Florida Statutes and/or contract enforcement remedies.

If Practitioner identifies a probable violation of the Xxxxx Law and repays the applicable Overpayment directly to the CMS contractor, then Practitioner is not required by this Section III.G to submit the Reportable Event to CMS through the SRDP.

  • Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion - Lower Tier Covered Transactions (a) The prospective lower tier participant certifies, by submission of this proposal, that neither it nor its principals is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in this transaction by any federal department or agency.
