Vendor Staff. All Vendor Staff providing services must be listed below (or on an attached sheet if additional space is needed). The Vendor shall provide only each individual's full name, no nicknames or abbreviations. Individuals requiring a Level II background check may be contacted by phone to provide their social security number for clearance.

Vendor Staff. Vendor will be expected to have the following staff available to perform the Services, such Services being provided on-site at a Citizens’ facility upon Citizens’ request. All Vendor Staff shall be properly trained, qualified, and have the requisite manufacturer’s certification to perform the requested Services. All Vendor Staff must comply with all reasonable administrative requirements of Citizens and all controlling laws and regulations relevant to the Services. Note: The following positions will be used for billing purposes. One person may qualify/serve under multiple positions. Citizens and Vendor will mutually agree on what position (and therefore hourly rate) is appropriate for a particular Service.
