Weekly Indemnity Insurance. (a) An insured employee who is totally disabled and unable to work due to sickness or an accident not covered by Workers’ Compensation, will be paid a weekly benefit commencing on the first day of absence due to an accident or hospitalization and the fourth scheduled day of absence due to a sickness. The amount of the benefit and the maximum period for which the benefit is payable shall be as set out in the following schedule: Years of Continuous Full-time Employment with the Company At Least But Less Than Amount of Weekly Benefit Maximum Period First of the month next following date of hire 3 years 75% of basic weekly wage 26 weeks after 3 years 85% of basic weekly wage 26 weeks The weekly benefit shall be determined based on basic weekly wage and service as of the date of commencement of disability. (b) It is not required that the insured employee be confined to home, but the employee must be under the regular care and attendance of a licensed physician during the period for which benefits are being claimed. (c) Certification of illness for loss of time benefits, up to a maximum of ten (10) weeks for each period of disability, will be accepted if completed by a licensed Chiropractor, Podiatrist or Oral Surgeon. (d) A disability resulting from the same cause as a previous disability will be treated as a continuation of the previous disability unless the employee has returned to work on a continuous full-time basis for at least fourteen (14) days.
