25 326 4. CONTRACTOR shall, at a minimum, provide the following DSH per month per FTE:

30 c. Transition Coordinator shall provide seventy (70) DSH per month or eight hundred 31 forty (840) DSH per year.
32 d. Service Coordinator shall provide fifty (50) DSH per month or six hundred (600) DSH 33 per year.
34 e. Career and Education Specialist shall provide fifty (50) DSH per month or six hundred 35 (600) DSH per year.
36 f. Wellness & Recovery Clinician shall provide fifty (50) DSH per month or six hundred 37 (600) DSH per year.
1 g. Peer Support Specialist shall provide twenty five (25) DSH per month or three hundred 2 (300) DSH per year.
3 h. Subcontracted Psychiatrist shall provide ten (10) DSH per month or one hundred 4 twenty (120) DSH per year.
5 i. Subcontracted Sexual Offender Therapy shall provide twenty (20) DSH per month or 6 two hundred forty (240) DSH per year.
7 j. CONTRACTOR understands and agrees that this is a minimum standard and shall 8 make every effort to exceed this minimum.
9 4. CONTRACTOR shall maintain an ongoing active and on-going minimum caseload of two at least four 29 hundred twenty 10 sixty-seven (220467) unduplicated Clients/Client families , of which at least four hundred sixty-seven (467) shall be 30 Medi-Cal, throughout the term of the Agreement, unless otherwise 11 approved by ADMINISTRATOR.
12 31 5. CONTRACTOR shall provide a minimum of one (1) weekly Face-to-Face Contact with 13 Clients/Client families per year for FSW services.
14 6. CONTRACTOR shall ensure a Face-to-Face Contact weekly for every Client and/or their 15 family admitted to the program, unless written exception is granted by ADMINISTRATOR.
16 7. CONTRACTOR shall provide Face-to-Face Contact within three (3) business days of 17 Client’s Referral for services.
18 8. CONTRACTOR shall not refuse Client referrals if any of CONTRACTOR’s staff are 19 below 32 workload standards, as defined in the Staffing Paragraph, Subparagraph M. VI.J. of this Exhibit A to 20 the Agreement, unless 33 otherwise approved by ADMINISTRATOR.
