Agency error definition

Agency error means medical assistance incorrectly paid to or for the client because of action attributed to the department as the result of one or more of the following circumstances:
Agency error means a person or persons received benefits to which they were not entitled as a result of an error on the part of an eligibility worker at a local department of social services or the central processing unit.
Agency error means that an individual or household unintentionally received increased or decreased benefits caused by incorrect actions (including delayed or no action) by the Department of Human Services staff or department processes.
