Alternate physician means a physician licensed under A.R.S. Title 32 Chapter 13 or 17 who signs a drug therapy management agreement to temporarily assume responsibility for supervision and evaluation of the drug therapy man- agement performed by a pharmacist when the supervisory physician is unavailable by direct telecommunication or physical presence at the practice site.
