Authorized health care provider definition

Authorized health care provider means an individual who is licensed by the State of California to prescribe medication.
Authorized health care provider means a health care professional who meets the definition of attending physician, as provided in Oregon workers’ compensation laws.

Authorized Health Care Provider shall not include a person or entity that provides services in the following settings: physicians or physician’s offices, senior independent living or retirement facilities, food and drug wholesalers/retailers, distributors and other similar resellers, retail pharmacies, retail stores, and other similar entities. Further, Authorized Health care Provider shall not include companies that provide home delivery services, Internet ordering and delivery services, or similar services, unless such services are provided as a part of an Authorized Health Care Provider’s Own Use. Products will not be purchased for distribution in any manner that would compete in the retail marketplace. In addition, Products purchased under this Agreement shall not be resold, provided to patients at time of discharge, or provided to any employee of Member or any other entity or person outside of the Own Use requirements defined herein.
