Benefit Package definition

Benefit Package means the package of covered health care services for which the client is eligible.
Benefit Package means the covered services described in Appendix K of this Agreement to be provided to the Enrollee, as Enrollee is hereinafter defined, by or through the Contractor.
Benefit Package means the covered services for the MMC and/or FHPlus Programs, described in Appendix K of this Agreement, to be provided to the Enrollee, as Enrollee is defined in this Agreement, by or through the Contractor, including optional Benefit Package services, if any, as specified in Appendix M of this Agreement.

Examples of Benefit Package in a sentence

  • CMS required transition supply, as a minimum (unless prescriptions are written for fewer days); the supply is determined as the number of days submitted for the Plan Benefit Package (PBP)’s applicable month’s supply submitted to CMS for the relevant plan year.

  • Services in the Long-Term Care Benefit Package The MCO may use the Resource Allocation Decision Method (RAD) as its service authorization policy.

  • CMS will provide technical instructions and guidance upon release of the HPMS formulary and bid functionality as well as the Plan Benefit Package (PBP) and Bid Pricing Tool (BPT) software.

  • In other words, unless an exception applies, the Participant may not change any elections for the duration of the Period of Coverage regarding: - participation in this Plan;- salary reduction amounts; or- election of particular Benefit Package Options (including the various Health FSA Options).

More Definitions of Benefit Package

Benefit Package means the set of health care service categories included in a client's health care program. See WAC 182-501-0060.
Benefit Package means the array and type of services, as described by program-specific rules, for which the recipient is eligible.
Benefit Package has the meaning provided for in OAR 410-120-0000.
Benefit Package means Medicaid Covered Services, including home and community-based services, which shall be furnished by the CONTRACTOR. [See, NMAC 8.307.7, 8.310.2, 8.311.1, et seq.].
Benefit Package means Medicaid covered services, which shall be furnished by the MCO and for which payment is included in the capitation rate.
Benefit Package means the covered health care services described in section 2 of this Appendix to be provided by or through the CONTRACTOR to the enrollees.
Benefit Package means the covered services described in Appendix K of this Agreement, to be provided to the Enrollee, as Enrollee is defined in this Agreement, by or through the Contractor, including optional Benefit Package services, if any, as specified in Appendix M of this Agreement.
