Covered Expense definition

Covered Expense means expense incurred only for the following:
Covered Expense means expense incurred for services and supplies: (a) listed below; and (b) ordered or prescribed by a Legally Qualified Physician as Medically Necessary for diagnosis or treatment; which is limited to:
Covered Expense means a Usual and Customary fee for a Reasonable, Medically Necessary service, treatment or supply, meant to improve a condition or participant’s health, which is eligible for coverage in this Plan. Covered Expenses will be determined based upon all other Plan provisions. When more than one treatment option is available, and one option is no more effective than another, the Covered Expense is the least costly option that is no less effective than any other option.

Examples of Covered Expense in a sentence

  • The remainder is the amount of Prescription Drug Covered Expense for that claim.

  • If you owe a Coinsurance amount, then the Claims Administrator will apply the Coinsurance percentage to the Prescription Drug Covered Expense.

  • The Manager shall provide to the Sub-Adviser reasonable proof of the amount incurred and that it is a Covered Expense and the Sub-Adviser shall provide reimbursement promptly after receipt of such proof.

  • A Covered Expense is “Incurred” on the date the service is rendered or the supply is obtained.

  • Services rendered in the care and management of a pregnancy for a Covered Person are a Covered Expense under this Contract as specified in the Schedule of Benefits.


More Definitions of Covered Expense

Covered Expense means a medically necessary service or supply which is usual, customary and reasonable, and which is listed for coverage in this Plan.
Covered Expense has the meaning set forth in Section 8.4.
Covered Expense means a Usual and Customary fee for a Reasonable, Medically Necessary service, treatment, or supply, meant to improve a condition or participant’s health, which is eligible for coverage in this Plan. Covered Expenses will be determined based upon all other Plan provisions. When more than one treatment option is available, and one option is no more effective than another, the Covered Expense is the least costly option that is no less effective than any other option. All treatment is subject to benefit payment maximums shown in the Summary of Benefits and as determined elsewhere in this document.
Covered Expense means expenses actually incurred by or on behalf of a Participant for treatment, services and supplies covered by the Plan from a Provider. An eligible medical expense is deemed to be incurred on the date such treatment, service or supply, that gave rise to the expense or the charge, was rendered or obtained.
Covered Expense means Reasonable Charges in excess of the Government Health Insurance Plan of the Canadian province or territory in which You reside or any private or provincial or territorial Auto Insurance Plan for supplies, treatment or services listed in The Benefits section subject to policy limitations.
Covered Expense means Reasonable Expenses in excess of the Government Health Insurance Plan of the Canadian province or territory in which You reside, any other Insurance Plan with the same or similar coverage provided under this policy, or any private or provincial or territorial Auto Insurance Plan for supplies, treatment or services listed in The Benefits section subject to policy limitations.
Covered Expense means a charge for a service or supply allowable under the applicable Benefit Plan.
