Enrolled Patient definition

Enrolled Patient means a Client’s patient, such patient’s family, healthcare providers and other third parties authorized by Client to use the Application for such Enrolled Patient’s benefit. An Enrolled Patient is not a party to this Agreement and may use this Application solely under the management of an Authorized User.
Enrolled Patient means a patient with whom the Clinic has reached an explicit agreement that the Clinic will be that patient’s provider of regular, ongoing primary care, and which outcome has been documented by the Clinic in its EMR as described in Appendix 3. Enrolment of patients is optional for the Clinic. “Interprofessional Team Demonstration Initiative (ITDI)” means the provincial initiative, participation in which is governed by this Agreement;
Enrolled Patient means an Insured Person who is enrolled with a FHN Physician under this agreement;

Examples of Enrolled Patient in a sentence

  • An Enrolled Patient is not a party to this Agreement and may use this Application solely under the management of an Authorized User.

  • All references to an Enrolled Patient are intended to include an Enrolled Patient's parent or guardian where appropriate.

  • Medicaid means that government-sponsored entitlement program under Title XIX, P.L. 89-97 of the Social Security Act, which provides federal grants to states for medical assistance based on specific eligibility criteria, as set forth on Section 1396, et seq. of Title 42 of the United States Code.

  • Participating Orthotic Provider means an Orthotic Provider who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Medicare benefit means the Medicare benefit payable within the meaning of Part II of the Health Insurance Act 1973 with respect to a professional service.

  • Non-Participating Orthotic Provider means an Orthotic Provider who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Non-Participating Prosthetic Provider means a Prosthetic Provider who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Population means the population as ascertained at the last preceding census of which the relevant figures have been published;

  • Potential Enrollee means a Medical Assistance Recipient who may voluntarily elect to enroll in a given managed care program, but is not yet an Enrollee of an MCO.

  • Intensive Care Unit means an identified section, ward or wing of a hospital which is under the constant supervision of a dedicated medical practitioner(s), and which is specially equipped for the continuous monitoring and treatment of patients who are in a critical condition, or require life support facilities and where the level of care and supervision is considerably more sophisticated and intensive than in the ordinary and other wards.

  • Covered service means a health care service provided to a
