Safety/health: Fatality or life-threatening illness/disease (e.g. mesothelioma) to one or more persons.
When investigating an OICI involving a motor vehicle, the Protocol Team Leader shall be entitled to call out its or another agency’s Traffic Fatality Investigation Team for purposes of assisting the OICI investigation.
In the case of a Fatality the Contractor will also inform the Authorities (Department of Labour or the Department of Minerals and Energy in RSA) for their investigation.
In order to receive funding through the CJA Grant, which also supports the Attorney General’s Task Force on Child Abuse and Neglect, the State is required by statute to establish a child fatality review panel “to evaluate the extent to which agencies are effectively discharging their child protection responsibilities.” The New Hampshire Child Fatality Review Committee meets the criteria for this review panel (Appendix E).
