Former Youth in Care definition

Former Youth in Care means a youth under the age of 21 who was previously under the legal custody or guardianship of DCFS but was reunified with their biological family, was adopted, was placed in subsidized guardianship, or whose Juvenile Court case was closed and is no longer under the legal custody of DCFS.
Former Youth in Care means any child under the age of twenty-one (21) who was previously a Youth in Care and has been adopted or awarded a guardianship. Referred to as “DCFS Youth” when referenced in combination with “Youth in Care”.

Examples of Former Youth in Care in a sentence

  • The initial risk adjustment period shall be the following time periods for the DCFS populations during CY 2020: for DCFS Former Youth in Care, February 2020 through June 2020; and for DCFS Youth in Care, September 2020 through December 2020.

  • Risk adjustment for the second half of CY 2020 for the Former Youth in Care population will be weighted based on July 2020 enrollment.

  • For purposes of this section 5.28, the following terms shall e later of (i) thirty (30) days following (A) the Effective Date (in respect of Former Youth in Care) or (B) the Comprehensive Implementation Date (in respect of Youth in Care), as applicable, and (ii) the date on which Department provides all information reasonably necessary for Contractor to submit such reports and information; months of the State Fiscal Year beginning with the first (1st) day of July.

  • For the DCFS Youth Managed Care Specialty Plan, enrollment of DCFS Youth in Care Prospective Enrollees will begin on the Comprehensive Implementation Date, and enrollment of Former Youth in Care will begin on February 1, 2020.

  • The Department and Contractor enter into this Contract in order to deliver integrated and quality managed care to DCFS Youth in Care and Former Youth in Care Enrollees (together, the “Covered Enrollees”).

  • The population categories that will be risk- adjusted are DCFS Youth in Care and DCFS Former Youth in Care.

  • Drop-in care means the care of children on a one-time, occasional, or unscheduled basis to meet the short-term needs of families.

  • Child care means continuous care and supervision of five or more qualifying children that is:

  • Nursing Care Plan means a plan of care developed by a nurse that describes the medical, nursing, psychosocial, and other needs of a child and how those needs shall be met. The Nursing Care Plan includes which tasks shall be taught, assigned, or delegated to the qualified provider or family.

  • Homecare Worker means a provider, as described in OAR 411-031- 0040, that is directly employed by a consumer to provide either hourly or live-in services to the eligible consumer.

  • Nursing care means the practice of nursing by a licensed nurse, including tasks and functions relating to the provision of "nursing care" that are taught or delegated under specified conditions by a registered nurse to a person other than licensed nursing personnel, as governed by ORS chapter 678 and rules adopted by the Oregon State Board of Nursing in OAR chapter 851.

  • Pharmacy benefits management means the administration or management of prescription drug

  • Home Health Care means the continual care and treatment of an individual if:

  • Pharmacy care means medications prescribed by a licensed physician and any health-related services considered medically necessary to determine the need or effectiveness of the medications.

  • Skilled Nursing Care means services requiring the skill, training or supervision of licensed nursing personnel.

  • Medically necessary care means care that is (1) appropriate and consistent with and essential for the prevention, diagnosis, or treatment of a Patient’s condition; (2) the most appropriate supply or level of service for the Patient’s condition that can be provided safely; (3) not provided primarily

  • Health care worker means a person other than a health care professional who provides medical, dental, or other health-related care or treatment under the direction of a health care professional with the authority to direct that individual's activities, including medical technicians, medical assistants, dental assistants, orderlies, aides, and individuals acting in similar capacities.

  • Nursing program means a series of courses leading to an LPN certificate or an ADN degree provided through a Wyoming community college; or a BSN degree or a Master’s degree provided through the University of Wyoming; or, in the case of a candidate for a doctoral degree, provided through an institution offering the required nursing education courses under contract with the WICHE, or in a distance nursing education program provided through a university that is regionally accredited.

  • IMDG Code means the International Maritime Dangerous Goods Code, for the implementation of Chapter VII, Part A, of the International Convention for the Safety of Life at Sea, 1974 (SOLAS Convention), published by the International Maritime Organization (IMO), London;

  • Family and Medical Leave means a leave of absence for the birth, adoption or foster care of a child, or for the care of your child, spouse or parent or for your own serious health condition as those terms are defined by the Federal Family and Medical Leave Act of 1993 (FMLA) and any amendments, or by applicable state law.

  • Certified Domestic Violence Specialist means a person who has fulfilled the requirements of certification as a Domestic Violence Specialist established by the New Jersey Association of Domestic Violence Professionals.

  • Youth ’ means an individual between the ages of 18 and 35.

  • Information Technologies means all information processing and communications-related hardware, Software, supplies, and consumable items that the Supplier is required to supply and install under the Contract.

  • Uniform Code means the New York State Uniform Fire Prevention and Building Code, as currently in effect and as hereafter amended from time to time.

  • Health Insurance Portability and Accountability Act means the Health Insurance Portability and Accountability Act of 1996, Pub. L. No. 104-191, 110 Stat. 1936, as amended.

  • CHAMPUS means the Civilian Health and Medical Program of the Uniformed Service, a part of TRICARE, a medical benefits program supervised by the U.S. Department of Defense.

  • Consumer fireworks means fireworks devices that are designed to produce visible effects by combustion, that are required to comply with the construction, chemical composition, and labeling regulations promulgated by the United States consumer product safety commission under 16 CFR parts 1500 and 1507, and that are listed in APA standard 87-1, 3.1.2, 3.1.3, or 3.5. Consumer fireworks does not include low-impact fireworks.

  • Records management means the systematic control of all records from creation or receipt through processing, distribution, maintenance and retrieval, to their ultimate disposition.
