Tenancy Sustaining Services may include the following, as needed: • Individualized service planning with individuals to review, update and modify housing support and crisis plan to reflect current needs and address existing or recurring housing retention barriers.

For all Projects including Supportive Housing units, points will be awarded for experience in the last ten years providing intensive case management (Individualized service planning and the provision of connections to mental health, substance abuse, employment, health, housing retention and similar services) to homeless populations in supportive or transitional housing, and for demonstrated expertise working with homeless veterans.
