Person-Centered Service Plan means the details of the supports, desired outcomes, activities, and resources required for an individual to achieve and maintain personal goals, health, and safety, as described in OAR 411-004-0030.

Person-Centered Service Plan means a document that identifies a client's goals and assessed health and welfare needs. The person-cen- tered service plan also indicates the paid services and natural sup- ports that will assist the client to achieve their goals and address their assessed needs.
