Molina Medicare Choice Care (HMO) • Have both Medicare Part A and enrolled in Medicare Part B;• Permanently reside in Molina’s geographic service area, which includes the following counties: Ada, Canyon, Twin Falls;• Member or Member’s legal representative completes an enrollment election form completely and accurately;• Is fully informed and agrees to abide by the rules of Molina Medicare; and,• The Member makes a valid enrollment request that is received by the plan during an election period.
