Plan Design Document definition

Plan Design Document means the benefit document prepared by the Contractor in conjunction with the County and approved in writing by the County, which is used by the Contractor in processing prescription drug claims in connection with this Contract.
Plan Design Document means various documents or forms, including implementation forms, clinical management forms, clinical utilization or other documents, prepared by Caremark and approved by Participating Group, as may be modified by Participating Group from time to time in accordance with Section 6.3 of this Agreement, which detailthe relevant parts of the Plan for prescription drug benefits and clinical programs adopted by Participating Group and which are used by Caremark to provide Services under this Agreement.
Plan Design Document or “PDD” means the document prepared by Caremark and approved in writing by Participating Group that may be amended by Participating Group, from time to time, in accordance with this Agreement, which documents the relevant parts of Participating Group’s Plan for prescription drug benefits and which is used by Caremark to process Claims under this Agreement.

  • Design Development Documents means the Drawings, Specifications and other documents prepared by the Trade Contractor that establish and describe the size and character of the Trade Contractor Work as to architectural, civil, structural, mechanical and electrical systems, graphics and signage, and other elements, and which include typical construction details, equipment layouts and specifications that identify major materials and systems.

  • Application Documents means collectively: (i) the grant application submitted by Grantee, including all exhibits, schedules, appendices and attachments thereto; (ii) all documents, correspondence and other written materials submitted with respect to the grant application; and (iii) all amendments, modifications or supplements to any of the foregoing approved in writing by City.

  • State Standard Agreement means the form used by the State to enter into agreements with other parties. Several originally signed, fully executed versions of the State Standard Agreement, together with the integrated Contract Documents, shall each represent the Agreement as an individual “Contract Counterpart.”

  • Statement of Work means the description of activities performed in completing the Project, as specified in the Contract and as may be amended.

  • EHC plan means an Education, Health and Care plan made under sections 37(2) of the Children and Families Act 2014.

  • Construction Documents means the plans, specifications, approved change orders, revisions, addenda and other information approved by the City, which set forth in detail the Work to be performed for a construction Project.

The Design Criteria Package must specify performance- based criteria for the public construction project, including the legal description of the site, survey information concerning the site, interior space requirements, material quality standards, schematic layouts and conceptual design criteria of the project, cost or budget estimates, design and construction schedules, site development requirements, provisions for utilities, stormwater retention and disposal, and parking requirements applicable to the project. Design Criteria Packages shall require firms to submit information regarding the qualifications, availability, and past work of the firms, including the partners and members thereof.

  • Subproject Agreement means any of the agreements referred to in Section 3.07 of this Agreement; and
