Student health record definition

Student health record means documented information relevant to the health of the student to manage the routine and emergency care of the student while school is in session.
Student health record means a record containing the immunization status of a student relating to the vaccine-preventable diseases covered by this Part. The content of the immunization record form usedutilized by an institution shall include, as a minimum, the basic elements listed in Appendix C.
