A large quantity handler who manages recalled universal waste pesticides as described in §§ 7-903(a)(1) and (a)(2) and who has sent notification to EPA as required by 40 CFR Part 165 is not required to notify for those recalled universal waste pesticides under this section. A notification submitted by a large quantity handler must include: The large quantity handler's name and mailing address;The name and business telephone number of the person at the large quantity handler's site who should be contacted regarding universal waste management activities; The address or physical location of the universal waste management activities; A list of all of the types of universal waste managed by the large quantity handler;A statement indicating that the large quantity handler is either accumulating 5,000 kilograms or more of universal waste other than CRTs, or 36,288 kilograms (40 tons) or more of CRTs, at one time and the types of universal waste the handler is accumulating above this quantity.
