After reviewing a Donated Sick Leave Request for a transfer of sick leave days from the sick leave bank, the committee shall determine by a vote of it members whether the request is to be accepted or denied. If a majority of the members of the committee vote in favor of accepting the request for the transfer of sick leave days from the “Sick Leave Bank”, the committee shall communicate the recommendation to the requesting district employee. The decision made by the committee shall be considered final. In the event that the majority of the committee members do not vote to accept the request, it shall be deemed to be denied and the requesting employee will be notified in writing.

The Report must contain the following fields: • Participating Entity Name (e.g., City of Staples Highway Department); • Participating Entity Physical Street Address; • Participating Entity City; • Participating Entity State/Province; • Participating Entity Zip/Postal Code; • Participating Entity Contact Name; • Participating Entity Contact Email Address; • Participating Entity Contact Telephone Number; • Sourcewell Assigned Entity/Participating Entity Number; • Item Purchased Description; • Item Purchased Price; • Sourcewell Administrative Fee Applied; and • Date Purchase was invoiced/sale was recognized as revenue by Supplier.
